You (the Borrower) can apply for forgiveness of your Paycheck Protection Program (PPP) loan using the SBA Form 3508S.
If borrowers do not apply for forgiveness within 10 months after the last day of the covered period, they may be required to begin loan repayment as per the SBA PPP note.
